一、活动主题

践行师德、携手育人

二、工作目标

“课外访万家”活动，要求所有教师全部参与。教师家访以实地走访为主要形式，班级家访以班主任为主，其他科任教师都要参加，覆盖全部学生家庭。通过家访，增进学校与家庭、学校与社会、教师与家长、教师与学生之间的理解，增强育人意识、责任意识、共促教育和谐发展。

三、活动任务

1、了解学生家庭基本状况和在家的学习习惯、生活习惯等；

2、向家长反馈学生在校学习情况及表现，与家长共同探讨促进学生发展的教育措施和方法，发现好的家庭教育典型；

3、对家长关注的热点问题做好宣传解释工作，赢得家长对学校工作的理解和支持；

4、宣传有关学生健康成长的心理、生理、安全等方面与知识；

5、听取家长对学校工作的意见和建议。

四、活动要求

家访要面向全体学生，特别是突出重点，做到“六回访”；要落实家访内容，做到“六必进”；要讲求实效，做到“六知道”；要遵守纪律，做到“六不准”。

一是实行“六回访”：即生活有特殊困难的学生家庭，学习困难的学生家庭，留守儿童家庭(单亲家庭)，思想、学业上有重大变化的学生家庭，进城务工子女家庭，随班就读残疾学生家庭，在此次实地走访的基础上，建立跟踪回访制度。

二是实施“六必进”：即教育的政策措施进家庭，贫困生资助方案进家庭，学困生转化措施进家庭，家庭教育指导进家庭，心理生理健康辅导进家庭，教师个性化关爱进家庭。

三是做到“六知道”：学生知道教师家访的目的和意义，家长指导学生在校学习表现情况，家长知道学生的任课教师情况，教师知道学生的家庭情况成长环境，教师知道家长对孩子的希望和期盼，学校知道家访收集的家长意见和建议。

四是坚持“六不准”：不准要求家长安排车辆接送，不准借家访名义向家长提出任何私人要求，不准收受家长馈赠的钱、物，不准接受家长的吃请和报销开支，不准参与家长组织的娱乐消费活动，不准利用知悉或者掌握的招考等教育信息向家长谋取利益或以家访名义进行有偿家教。

五、活动安排

建立家长访效机制，开展教师全员定期家访活动，每学期每个学生家庭实地走访一次。今年集中家访活动为期4个月，从2011年9月开始，至12月底结束。集中家访活动分为三个阶段。

（一）宣传发动阶段（9月）

组织动员。9月开始，全校召开动员大会，进行工作部署。

分级培训。开展班主任和科任教师培训，明确家访活动的目的、意义、内容、方法和要求。

（二）推进实施阶段（9月——11月）

1、摸清信息。9月上旬，收录学生地址、家庭情况、联系电话等相关信息，进行分类整理。拟定班级家访计划。

2、明确任务。9月中旬，学校结合实际，明确工作目标，落实工作职责，分解工作任务。制订实施方案，有计划、有步骤地开展活动。

3、开展家访。家访是学校德育工作的一项重要工作，也是班主任工作和教师教书育人的重要内容。家访以班级为单位，班主任带头家访，科任教师全员参与。家访后，对学生家庭状况、校内校外表现进行对比分析，对学生心理状况进行评估，认真填写《家访手记》（样式见附件）。通过文字、图片等形式建立学生家访档案。家访活动结束后，每个家访的教师要写出一份家访情况报告。

（三）总结表彰阶段（12月）

1、工作总结。12月1日至10日，全校对集中家访活动进行自查总结，整理家长的意见和建议，分析社会家庭育人的特点，提出改进教育方式的意见和建议，并撰写总结报告。

2、活动考评。12月11日至25日，对家访活动组织开展情况进行考评、总结。考评情况将作为班主任、教师年度目标考核的一项重要内容。

3、表彰奖励。在认真总结的基础上，表彰一批先进个人。
